rib, m1L and m1r increase confusion!

This is my first post in this forum, nice to meet all of you lovely knitters!
I came here because I’m relatively prone to pattern confusion. I’ve been working on my first-ever sweater and I finished the collar, then got to the shoulder increase. I followed the pattern (which i’ve attached a photo of)

When I make the new stitch, I don’t include it as part of the 2x2 knit purl, rather I do a knit, knit, increase, purl, purl, or purl, purl, increase, knit, knit. I include the newly made stitch in the next row. However, the rib pattern didn’t continue in its columns. The columns seem to be moving over one stitch every row, which is making this weird diagonal design (I’ve also attached a picture below).

I kept trying to sort of restart mid project by following the columns which were there before, just to see if that might fix it. Here’s some pictures of that. Anybody know what I might be doing wrong?

Yes, the additional sts will have to be worked as knit one or purl one sts until you have enough increases to incorporate them into the k2p2 rib. Keep working them as either knits or purls but do not let them throw off the rib pattern. Even if you have an increase as p1 before the k2p2, that’s ok. On the next increase row, it’ll be p2 before the k2p2 rib.

What is the name of your pattern?
