Pattern is from Debbie Bliss book called Angel and the pattern is ‘Big shrug’ on page 21
After co 102sts knitting 1st rib row p2,k2,p2
the 2nd rib row p2,k2,p2 here is where I’m lost
Place markers at each end of last row.
Next row cast on 44sts k to end.
Do I cast on 44sts onto the 102 sts? then k to end??? what end?
Next row cast on 44sts p to end total 190sts.
Where do I cast on?

You’re adding 44 stitches to each side of the central 102.
What it looks like to me is that you’ll use new yarn and cast on 44 stitches. Using that yarn, you’ll knit across the 102.
On the next, wrong side, row, cast on 44 and then purl across the 102 + 44.
You don’t need to use another yarn end, just take the working yarn and add the sts with the knit or cable CO. Then knit them and the orginal sts to the end of the row. Turn and CO sts at the beginning of that row.
