Confused- knitting a collar on a jacket

I’m knitting the collar on a women’s fitted jacket (Wendy 5405), and I just can’t make sense of it.
You knit collar after the front, sewing it in place as you go. Half way up, the lapel of the collar indents in and the out again making a > type shape on the ouside edge.

With 14 sts on the needle, the pattern says:
At the outer edge, cast off 5 sts, then cast on 5 sts, pattern to end (14sts)
work 3 rows, ending at outer edge.

I just cannot understand how this works. I’ve tried several times.
Is this correct and if so, how do you cast on 5 stitches mid-row?

You can cast on using a cable cast on or a knitted cast on. Turn your work so that you’ll be casting onto the needle in your left hand. There are videos of both if you go to the Free Videos tab at the top of this page and look under Cast On. They’re really perfect for this kind of notched collar.

This helps to make sense of the casting on, but then it creates a hole between the rows? Do I just stictch that up later?

I don’t think there should be a hole. Do you cast off 5 and work to the end of the row (9sts), turn, work those sts and at the end of this second row cast on 5sts? or is it cast off and immediately cast on. I tried cast off 5 sts, then immediately cast on 5 sts I don’t seem to leava a gap but if you have one, I think you can close it up later.
