After you knit the 3 from your left needle, you sort of pull/push the cable needle to the left and knit from it as though it were a regular needle onto the right needle… Then when the stitches are knitted off, you resume knitting from your left needle. From the way I read the pattern, you would knit the stitches normally. The “Hold in back” only refers to you holding the stitches on the cable needle to the back of your work while you knit the next 3 stitches.
