Row 5, bind off for buttonhole: K4, then pass 3rd st over 4th st just knitted , K 25
Row 6, cast on for buttonhole:K 26, turn to right side, knit 1 in stitch on left hand needle, turn and K 2.
Rows 7 through 12 (6 rows) Work even.

Is it the directions for the buttonhoe that are causing the problem?
On row 5 you decrease one stitch by passing the 3rd stitch over the 4th (the one closest to the needle tip)
On row 6, you cast on one stitch to make up for the one bound off and complete the buttonhole. You need to switch the needles in your hands so that the working yarn is coming off the needle in your left hand. Knitting one in the stitch on the lefthand needle is a knit cast on, like this.
Then switch the needles in your hands again so that the working yarn is coming off the needle in your right hand and k2.
