Never seen this before...what does it mean?

This is a sweater pattern.

Book “Textured Knits” reads:

Sizes

                           small/medium.........       medium/large

To fit bust cm (in) 86-94 (34-37)… 97-107 (38-42)

Actual size 104 (42) … 122(48)

I do not understand the… Actual size…does this mean that in the large size for example the sweater will measure 48" around?

The directions for the front read "cast on 70 (78) sts Chunky yarn.
12 sts x 16 rows = 4 x 4 in size 11 needles.

Not positive, but I think that means the actual garment measurements when it’s finished.

Yes, it sounds like it means the finished garment measures 48" for the large so you have quite a bit of ease added in for the 38"-42" bust it is supposed to fit.
