Need help with the M1 stitch

When I do the M1 stitch I always go through the back of the loop to prevent a hole in the work but I still seem to be getting holes even though I go through the back of the loop.

Please can anyone advise if there is a proper technique to prevent a hole?

Are you twisting the loop when you knit into it?
