It means you should get 20 sts across a 4" measurement. The goal is to find out how many sts per inch (or 4"), not to make a 4" square. It’s better to cast on at least 4 or 6 more sts, then count the sts in 4" across the middle without including the edge sts. They’re a little different than the ones in the middle and will curl under.
