insert needle down between 4th and 5th stitch???

Another obstacle in my knitwork Drops 179-2.: In the Diagram you see a mark over 4 stitches and the explanation sais: insert needle down between 4th and 5th stitch, pull yarn through (= 1 new stitch), knit 1, purl 2, knit 1.
Does it mean you have to turn around some stitches, when you have to go first to the 4th stitch?

It’s like the smocking stitch shown in this video.


For your pattern you put the needle between the 4th and 5th sts but it’s a similar idea. Your pattern then works those sts as knits and purls rather than using a k2tog.
