How do you cast on Mid Row?

I am knitting Chic Knits Mondo Cable Shell/Vest, and it says:

With WS facing, sl 11 st from provisional cast on of back left shoulder to one point of larger circular needle. (I did that).
NOW, it says :

Attach working yarn and CO 7 stitches - 18 sts. Does this mean I cast on 7 more stitches to the 11 I just put back on the left needle? Do I start with one of those 11, or cast on 7 new stitches which will be knitted together in the next row??:think: :oo: :???:

Take the needle with the 11 transferred sts in your left hand and use a knitted or a cable cast on to CO [I]7 additional [/I]sts. You can see both cast ons in the Free Videos tab at the top of this page under Cast Ons.
