following 6th rows?

This is the pattern instruction (KING COLE 4745) - edited to just show for my size.

1st row (rs). K60, p17, k60

2nd row. Knit.

3rd row. (p1, k1) 30 times, p17, (k1, p1) 30 times

4th row. (k1, p1) 30 times, k17, (p1, k1) 30 times

From 1st to 4th row forms patt and centre rev st-st vent.

5th row. K60, p2tog, p13, p2tog, K60 [135] sts.

6th row. Knit.

7th row. (k1, p1) 30 times, p15, (k1, p1) 30 times

8th row.k1, p1) 30 times, k15, (p1, k1) 30 times

Cont in patt dec 2 sts in centre vent as before in next and every foll 6th row to 105[111:117:123:127] sts.

I’m confused.. This appears to be a 4 row repeat.

According to this, the decrease happens in the first row of the repeat, or would I do the decrease on other rows?

So would I decrease on row 5, 9, 13 , 17 , 21 25,19 (ie first row of the 4 row repeat)

OR

row 5, 11, 17, 23, 29, 35, 41

Decrease on rows 5,11,17,23,29,35 and 41 would be correct. Work the following rows without decreases including row 5. Work decreases just on this second set of rows as you posted.
The decrease in the “next” is confusing here but there doesn’t seem to be a good reason for a more rapid decrease here (i.e. row 5 and then row 9, then every 6th).
