I am up to the _”Shape sleeve top” part of the pattern in my crew neck sweater, and I understand up to the bit where it says “Dec 1st as set at each end …. “ etc.
Does this mean decrease the first and last stitch on next and then every 4th row, and if so, does the “as set” refer to skpo and k2tog? If that is how I decrease, do I do that on the first and last two stitches, or do I K2 first and K2 at the end?

This pattern is from a Dorling Kindersley book for beginning knitters.
Dec 1st as set…
The “as set” instruction is telling you to follow the placement and stitch shown in the first Next Row under the Shape sleeve top. The pattern is using a skpo and k2tog, each 2sts in from the edge which gives you a nice edge for seaming. [If you prefer to reverse the decreases (begin with k2tog and end with skpo) or use other decreases or placement you can of course.]
So you would work the initial decrease, call it row 1 and then decrease on rows 3,5 and 7 for the smallest size for example then on rows 9,13,17,21,25 and 29. Then proceed to the next row with double decreases at each end

I’m doing next size up (M), so I think I would do the initial decrease (“row 1”), then on rows 3, 5, 7 & 9, then 11, 15, 19, 23, 27 & 31. I’m hoping that is correct.

3, 5, 7, 9 are correct for the second size.
Then 13, 17, 21, 25, 29 for the 5 following 4th rows

I was thinking that “…after my initial decrease (let’s call it row 1), I would do the following:
Row 2 purl
Row 3 decrease
Row 4 purl
Row 5 decrease
Row 6 purl
Row 7 decrease
Row 8 purl
Row 9 decrease
Row 10 purl
Then the “next” row (as in “…at each end of next and 5 foll 4th rows) is row 11, which is a decrease row.
This would be followed by decreases every 4 rows five times, making that rows 15,19,23,27 & 31.
I think that’s what the first reply was saying, except that I’m doing the second size.
