Hi I’m confused on this part of the pattern
Cont on last set of 40 sts only for left
front.
Work a further 18 rows, so ending
at sleeve edge.
Does this mean I knit and purl fir a further 18 rows for the left hand side only (as I have the right handside on a separate needle and cast off the middle section)

Yes, work just on the left side. The line before the one you quoted says: Cont on last set of 34(40:45:51:56) sts [U]only for left
front[/U]. Work a further 16(18:22:24:26) rows, so ending
at sleeve edge.
At the end of the Shape sleeve instruction it tells you to go back to the right front sts.

Does this mean

row 4: knit row no cast of
continue st st for 34 further rows?
If your next row is a purl row, then yes, that is what it means.
Yes and I would cast off for an additional 2 alternate rows. All total, cast off 4 times 6sts or 24sts cast off. Then 34rows in st st.
or:
Purl row (the next row) cast off 6 stitches
row 2: knit row no cast off
row3: purl row cast of 6 sts
row 4: knit row no cast off
row 5 purl row cast off 6sts
row 6 knit row no cast off
row 7 purl row cast off 6sts
continue st st for 34 further rows
