Casting on in the middle of a row?

I am knitting the front of a cardigan and am attempting the button hole. I have knitted several rows and then on the last row I did I knitted to the last 4 stitches, cast off 2 stitches and knitted 1. The next row says to knit 2, cast on 2 stitches and knit to end of row. How do I cast on these 2 stitches?
