Casting off in the middle of row

My pattern is design A matinee coat.
K2.yfw k2tog k21 for the r front, cast off 12 stitches k43 including st on the needle for back.
Do I knit 2 stitches and cast off the first one of those or do I cast off the very next stitch ie stich 22?

The first choice: knit 2 more sts then lift the first over the second and off the needle. That lifted stitch counts a one. You’ll continue to 21 sts lifted up and off the needle with one stitch remaining on the right needle and 42sts on the left needle.

Typo. Continue to 12 sts lifted up and off
