I’m stuck at the dec 1 st at neck edge of next 4 rows. Would this be a decrease only at the end where the first decrease happens? Not at the end where the cast offs are, right?

I think this might help answer your question. I’ll use the smallest size as the example.
•You cast off 5 stitches at the armhole edge.
•You knit across the body of the sweater until there are 20 stitches left. This is the neck edge.
•Put the 20 stitches on a holder and you’ll knit them later.

Yes, that’s correct. The decrease on the next 4 rows occurs at the neck edge. That’s where you made the turn and the initial decrease. You’ll work until you have the given number of sts for your size on the right needle then turn, decrease one stitch and work to the end of the row. Cast off the number of sts for your size (5 for the smallest size) at the beginning of the row. This is the armhole edge.
Now finish the row to the last couple of sts and decrease again at the neck edge. Then repeat the sequence, decreasing at the neck edge and binding off at the armhole edge according to your size. You’ll keep going until you’ve decreased 4 times total at the neck edge and bound off the given number of sts at the armhole edge.

I’m making the 44-46 (xl). Thank you so much for your input. I don’t know why this didn’t make sense to me. Sometimes they seem to cram to many instructions in one sentence.
Editors definitely seem to save space by compressing the directions. It sometimes makes you tease out every last word, especially the small ones. Have fun finishing up this lovely project!
